Easily report IT security and privacy incidents using the new online form | Electronic News

Faculty, staff and students can now easily report IT security and privacy incidents, regardless of their college, campus or department, using a new Qualtrics form on the ITS website, DefendYourData.wvu.eduor the Health Sciences Center STI website.

You must enter your WVU login credentials to submit a report. You will then be contacted by WVU Information Security Services or the WVU Health Sciences Center Privacy and Security Team for additional information. Individuals who do not have WVU credentials should contact [email protected] to request an incident form.

Security incidents are defined in the IT Security Incident Response Policy, but examples include a lost or stolen laptop, a suspected ransomware attack, or a hacked device. Examples of privacy incidents include, but are not limited to, items such as: inadvertently sharing personally identifiable information such as protected health information or FERPA-protected student data with a person not allowed to see them.

Suspected spam and phishing attempts, or emails intended to steal your personal information should continue to be reported to [email protected]. Please forward suspicious emails as an attachment or use the Report Message or Phishing button in Outlook Mail.
