County Monroe is experiencing increasing levels of Covid-19 and significant spread in our community. We have seen a significant increase in the number of new daily cases over the past few weeks, and the number of cases continues to increase.
If cases continue to escalate, there may be times when the case investigation and contact tracing efforts of the Monroe County Health Department become strained or at full capacity and may not be able to reach out to people. positive cases within 24 hours.
To facilitate disease investigation and contact tracing efforts or for those who want to save time, we offer the option of online case interviews. To access the online form, go to healthymonroecowi.org/positive and click on the âCOVID-19 Interview Formâ link. This will bring positive cases to an online interview form. Once the form is submitted, individuals will receive an email detailing isolation and contact instructions with close contacts, as well as links to request isolation and quarantine letters.
All information collected complies with the Health Insurance Portability and Accountability Act (HIPAA) and remains confidential and accessible only to health service personnel. Those who have already received a phone call from staff at the Monroe County Health Department do not need to complete the online form.
